Autologous bone grafting for tibial defects in total knee arthroplasty

Abstract

Asymmetric bone loss from the tibia is a common finding in patient with advanced arthritis being treated with total knee arthroplasty. Such bone loss must be corrected when total knee arthroplasty is done if optimal function of the reconstructed knee is expected. The options for their treatment have included; increased bone resection, filling with cement, cement reinforced with screws, the use of metal wedges, custom prosthesis and bone grafting. The aim of this is paper is to review our experience with autologous bone grafting for medial defects. 10 total knee arthroplasties were implanted in severely deformed knees with large bony defects. The defect was filled with an autologous bone graft obtained from the same knee and fixed with two or three screws. The patients were reviewed during an average follow-up period of 22.8 months (range 8-48). All of the grafts were completely united. There was no evidence of graft collapse in five knees and five knees demonstrated evidence of minor graft resorption. The mean Knee Society functional score postoperatively was 82.5 points and knee score was 87.1 points. Despite the short-term follow-up , we believe that tibial autologous grafting is an acceptable solution to managing severe defects in total knee arthroplasty.

Total diz artroplastisi uygulamalarında tibial komponentin stabilitesinde. dolayısıyla başarısında sağlam bir tibial yüzeyin ve protezin yüzeye tam oturmasının önemi yadsınamaz. Ancak tamamıyla sağlam ve defektsiz bir tibial plato her zaman için sağlanamayabilir. Özellikle aşırı derecede varus ve valgus deformitesi olan artritik dizlerde asimetrik kemik defektleri sık gözlenir. Tibia proksimalinde oluşan bu defektlerin tamiri için pek çok yöntem ve teknik tanımlanmıştır. Bu teknikler; defektin distaline kadar tibial kesinin derinleştirilmesi, defektin sement, sement-vida, metal wedge ler (kamalar), custom protezler ve kemik greftleri ile desteklenmesi ve tamirini içerir. Çalışmamızda ileri derecede deformiteli 7 hastanın 10 dizine uygulanan primer total diz artroplastilerinin sonuçlarını retrospektif olarak değerlendirdik. 3 ü erkek 4 ü kadın olan 7 hastanın operasyon sırasında ortalama yaşı 73.1 yıl (65-80), takip süresi ise 22.8 ay (8-48) olarak tespit edildi. Tüm vakalarda femoral kesilerden kazanılan otogreftler 2 ya da 3 adet vida ile tespit edilmiştir. Takiben 3 vakada Howmedica Kinemax, 7 vakada ise Zimmer MG II tipi protezler ile total diz artroplastisi uygulanmıştır. Radyolojik olarak takip süreleri sonrasında tüm grenlerin kaynadığı, grenlerin besinde hiç rezorpsiyon olmadığı besinde ise minör rezorpsiyon olduğu gözlendi. Klinik değerlendirmede postoperatif ortalama 22.8 aylık takip süresi sonrasında Knee Society klinik değerlendirme formuna göre diz skoru 100 üzerinden ortalama 87. l puan (62-93), fonksiyon skoru ise 100 üzerinden ortalama 82.5 puan (55-90) olarak bulundu. Sonuç olarak uzun dönemli klinik sonuçların da gözlenmesi gerekmekle birlikte primer diz artroplastisinde tibial kemik defektlerinin tamirinde otogreft ile rekonstrüksiyonun yeterli ve başarılı bir yöntem olduğunu düşünüyoruz.
