Comparison of the results of total knee arthroplasty with and without patellar resurfacing

Abstract

Objectives: To evaluate our experience in patients who underwent total knee arthroplasty with or without patellar resurfacing.\nMethods: The study included 55 patients (45 females, 10 males; mean age 67 years; range 50-78 years) who underwent total knee arthroplasty with (group I, 31 knees) or without (group II, 37 knees) patellar resurfacing. Etiologic factors were posttraumatic arthritis in three, and idiopathic arthritis in 52 patients. Patellar resurfacing was performed in patients in whom grade 4 chondropathy was detected during operation according to the Outerbridge classification. Lateral retinacular release was performed in 23 patients. Evaluation was performed using the clinical scoring system of the Knee Society and a patient questionnaire. The mean follow-up period was 34.8 months (range 17-50 months). \nResults: In group I, the mean pre- and postoperative knee scores were 42 and 77.7, and function scores were 48.1 and 84.5, respectively. In group II, the mean pre- and postoperative knee scores were 43.3 and 77.1, and function scores were 57 and 85.9, respectively. Prepatellar pain was observed in one patient (5%) and in three patients (14%) with and without patellar resurfacing, respectively. No significant differences were detected with regard to activities such as getting in or out of a car, rising from a chair, swimming, and riding a bicycle.\nConclusion: Given the lack of more favorable results between patients with and without patellar resurfacing, and the difficulty in patients requiring patellar revision, it is concluded that patellar resurfacing may not be necessary other than in patients with grade 4 chondropathy.

Amaç= Gonartroz nedeniyle total diz protezi uygulanan olgularda patellar komponentin değiştirilip değiştirilmemesine ilişkin deneyimlerimiz sunuldu.\nÇalışma planı= Total diz protezi yapılan 55 hastanın 68 dizi değerlendirildi. Hastaların 45$i kadın, 10$u erkek idi %yaş ort. 67q dağılım 50;78 . Etyolojik neden üç hastada posttravmatik artroz, 52 hastada idyopatik gonartroz idi. Otuz bir dizde patella değiştirildi. Patellanın değiştirilme endikasyonu ameliyat sırasında Outerbridge sınıflamasına göre grade 4 kondropatisi olanlarda uygulandı. Yirmi üç dizde lateral gevşetme yapıldı. Hastalar ameliyat öncesi ve sonrasinda Diz Cemiyeti$nin diz ve fonksiyon skorları ve hasta memnuniyeti formu kullanılarak değerlendirildi. Ortalama takip süresi 34.8 ay %dağılım17;50 ay idi.\nSonuçlar: Patellası değiştirilen 31 dizde ameliyat öncesi ve sonrasi diz skorları sırasıyla 42 ve 77.7, fonksiyon skorları 48.1 ve 84.5 bulundu. Patellası değiştirilmeyen 37 dizde ameliyat öncesi ve sonrasi diz skorları sırasıyla 43.3 ve 77.1, fonksiyon skorları 57 ve 85.9 idi. Patellası değiştirilen grupta bir olguda (%5), değiştirilmeyen grupta ise üç olguda (%14) diz önü ağnsının devam ettiği görüldü. Arabaya girip;çıkma, sandalyeye oturup;kalkma, yüzme ve bisiklete binme ile ilgili sorulara verilen yanıtlarda iki grup arasinda anlamlı fark görülmedi.\nÇıkarımlar: Klinik sonuçlarda üstünlük görülmediğinden ve patella revizyonunun güçlüğü de düşünüldüğünde, grade 4 kondropatisi olan hastalar dişinda patella yüzeyinin değiştirilmemesinin daha doğru olduğu sonucuna varıldı.
