Abstract
Osteomyelitis and septic arthritis are diseases that need surgical management Debridement of infected ischemic and necrotic fissues, followed by sterilization of dead space and musculocutaneous flap, and closing with procedures like bone graft or extremity reconstruction constitute the basis of treatment For sterilization of the debrided area, continued irrigation system was used in addition to appropnate systemic antibiotics due to the special features of the infection. While mechanical cleaning was made by irrigation, antisepsis was provided with 1% povidone iodine solution (PV-I) Between the years 1985-1992 in the Orthopedics and Traumatology Clinic of Şişli Effal Hospital of health ministry, closed irrigation system with povidone iodine was applied following debridement in the treatment of 13 nonspesific septic arthritis, 21 acute and 10 chronic osteomyelitis. Irrigation was continued for one week in septic arthritis, and for 2-3 weeks in osteomyelitis. The follow up of the patients was between 6 and 81 months Full recovery was obtained in 35 patients Various treatment methods were used for 9 patients who didn t recover clinically.
Özet
Osteomyelit ve septik artrit cerrahi tedavi gerektiren hastalıklardır. Tedavide en önemli faktör enfekte ölü ve iskemik dokulann debridmamm takiben enfekte boşluğun çeşitli metodlarla sterilizasyonu ve kas flebi, kemik grefti ya da ekstremite rekonstrüksiyonu gibi yöntemlerle kapatılmasıdır (2, 10). Debride edilmiş bölgenin sterilizasyonunda kültüre uygun sistemik antibiyotik kullanılmasına ilaveten, enfeksiyonun özelliğinden dolayı devamlı irrigasyon sistemi de kullanılmaktadır. İrrigasyonla mekanik temizlik yapılırken %1’lik polividin iyot (PV-I) solüsyon ile de antiseptik etki sağlanmaktadır. 1985-1992 yılları arasında Sağlık Bakanlığı Şişli Etfal Hastanesi Ortopedi ve Travmatoloji Kliniği nde 13 nonspesifik septik artrit, 21 akut ve 10 kronik osteomyelit tedavisinde debridmanı takiben polividin iyot ile kapalı irrigasyon sistemi uygulanmıştır. İrrigasyona septik artritte 1 haftada, osteomyelitlerde 2-3 hafta süreyle devam edilmiştir. Hastaların takip süresi 6-81 ay arasında değişmektedir. 35 hastada tam iyileşme elde edilmiştir. 9 hastada klinik ve laboratuvar olarak iyileşme görülmemesi üzerine değişik tedavi metodları uygulanmıştır.
