Pes cavus: an analysis of epidemiologic and clinical features

Abstract

Objectives: To determine the epidemiologic and clinical features of pes cavus deformity.\r\nMethods: Footprints of 3,511 primary school children in Isparta, were investigated with the use of podoscope. All children with pes cavus footprints were further inquired for trauma history, footwear, and subjective complaints. Local examination was performed with regard to pes cavus. Lateral block and push-up tests were carried out depending on the severity of the deformities. A comprehensive neurologic examination was undertaken in each case.\r\nResults: Footprints showed pes cavus in 791 feet of 406 children (11.7%). Most of these patients (65%) had various complaints, mainly heel pain and fatigue. The variety of complaints increased both with age and the severity of deformities (p=0.019 and p=0.006, respectively). Moreover, the severity of deformities increased significantly with age (p=0.006). Analyses of footprints showed that 19.2%, 62.5%, 17.7%, and 0.6% of cases had 1st, 2nd, 3rd and 4th degrees of pes cavus, respectively. Although subtle neurological abnormalities were detected in 37% of cases, an etiologic factor could be demonstrated in only six patients.\r\nConclusion: Footprints of school children showed a high prevalence of pes cavus deformity.

Özet

Amaç: Pes kavus (PK) deformitesinin epidemiyolojik ve klinik özelliklerini belirlemek.\r\nÇalışma planı: Isparta il merkezinde bulunan dokuz ilköğretim okulunda toplam 3511 öğrenci muayene edildi. Öğrencilerin podoskop üzerindeki ayak izleri incelendi. Pes kavus ayak izine sahip olan tüm öğrenciler, travma hikayesi, ayakkabı giyimi ve olası şikayetlere yönelik sorgulandı; takiben PK ya yönelik lokal muayene yapıldı. Deformitenin derecesine göre lateral blok testi ve push-up testi uygulandı. Her olgu ayrıntılı nörolojik muayeneden geçirildi.\r\nSonuçlar: Değerlendirme sonucunda 406 olgunun 791 ayağında PK ayak izi saptandı (%11.7). Pes kavuslu olguların %65’inde çeşitli şikayetler saptandı. Yaş ile deformitenin derecesi arttıkça, şikayeti olan olgu sayısının artmasından ziyade, kişi başına düşen şikayetlerin sayısı/çeşitliliği arttı (sırasıyla; p=0.019, p=0.006). Ayrıca, yaş arttıkça deformitenin giderek ağırlaştığı saptandı (p=0.006). Olguların %19.2’si birinci derece, %62.5’i ikinci derece, %17.7’si üçüncü derece ve %0.6’sı dördüncü derece PK ayak izine sahipti. Olguların %37’sinde hafif derecede nörolojik kusurlar saptamamıza rağmen, altı olgu hariç, etyolojide rol oynayabilecek herhangi bir hastalığa rastlanmadı. \r\nÇıkarımlar: Ayak izine göre saptanan PK prevalansının oldukça yüksek olduğu bulundu.
