The effect of embryonic mesenchymal tissue on fracture healing

Abstract

Bone grafts are used most frequently beside other methods for problems occuring with healing after trauma or tumors and for repair of bone defects. Ashton et al. have studied bone marrow and demonstrated that both bone and cartilage of an embryonic nature are produced by differentiated marrow stromal cells, suggesting that the undifferentiated precursor cell of the postnatal marrow has the same potential as the undifferentiated mesenchymal cell found in early embryonal stages of development. Our aim in this study was to evaluate the efficacy of embriyonic mesenchymal cells as a solution for the abovementioned problems and this method has not been used until now. We performed fractures on both femurs of 16 Wistar rats and than treated one side with osteosynthesis only with intramedullary Kirschner wire, and the other side was treated after an intramedullary Kirschner osteosynthesis with a vertebral colon from rat embryo implanted on fracture line and this colon is rich in mesenchymal cells. So, we both intended to get larger amount of mesenchymal cells and gain the time in which mesenchym cells proliferate. The femurs of 2 groups of rat, were examined radiologically and histologically, at 6th and 8th weeks. On radiological examination, callus was further developed on the femurs in which graft was used as opposed to the other side. On histologic examination, callus was much mature and graft was blended with it on the grafted side. Union procedure was further developed in the site of graft. Besides, the union time has been found longer in comparison with closed fracture union time in rats. It might depends on either the lack of fracture hematom or the immune reaction. Our results of this study confirm that, the intracellular milieu, the milieu in which fracture takes place or other miscellaneous unknown factors have great effect in the union procedure while mesenchym cells transforme to chondroblasts. Mesenchym tissue which is transferred as greft, unite with callus at last and boost it as quality and quantity. The immune reaction which developes against to the transferred mesenchym tissue although is out of the scope of this study, is another important factor which is expected to effect the result. As a result, we concluded that embryonal mesenchymal cells are effective in filling bone defects, but a longer follow-up time is required for the evaluation of tissue rejection.

Özet

Travmatik yada tümöral sebeplerden sonra meydana gelen kaynama problemleri ve kemik defektlerinin tamirinde diğer yöntemlerin yanında en sık olarak kemik greftleri uygulanmaktadır. Ashton ve arkl. kemik iliğini kullanarak ve stromal hücrelerinin embriyonik karakterde kemik ve kıkırdak hücrelerine dönüştüğünü göstermiştir. Bu da farklılaşmamış postnatal kemik iliği hücrelerinin embriyonel gelişmenin erken safhasındaki farklılaşmamış mezenkim hücreleri ile aynı gelişme potansiyeline sahip olduğunu gösterir. Bizde bu çalışmada yukarıda bahsedilen sorunların halledilmesinde, şimdiye kadar denenmemiş bir yöntem olan embriyonel mezenkim hücrelerinin etkinliğini değerlendirmeyi amaçladık. Bu amaçla 16 adet Wistar cinsi sıçanın her iki femurunda da kırık oluşturulduktan sonra bir taraftaki kırıklara intramedüller Kirschner teli ile interkondiler çentikten girilerek osteosentez sağlanmış, diğer tarafta ise intramedüller osteosentezden sonra, sıçan embriyosundan mikroskop altında alınan mezenkim hücrelerinden zengin vertebral kolon kırık hattına implante edilmiştir. Böylece hem daha yoğun mezenkim hücresi sağlamayı, hem de mezenkim hücrelerinin proliferasyonuna kadar geçen süreyi kazanmayı planladık. Sıçanlar 2 gruba ayrılmış, 6 ve 8 haftalık takipler sonunda femurlar radyolojik ve histolojik incelemeye tabi tutulmuştur. Radyolojik olarak greft konan taraflardaki femurlarda daha ileri safhada kallus görüntüsü, diğer tarafa göre daha sıklıkla tespit edilmiştir. Histolojik incelemede de kollusun diğer tarafa göre greftli tarafta daha olgun olduğu ve konulan greftin kallus dokusu ile bütünleştiği görülmüştür. Kaynama prosedürü greft konmayan tarafa göre daha fazla ilerlemiş bulunmuştur. Bunun yanında kaynama süresi sıçanlardaki kapalı kırık kaynama süresi dikkate alındığında uzun bulunmuştur. Bu kırık hematomunun kaybına veya immün reaksiyona bağlı olabilir. Bu çalışmadaki sonuçlarımız gösteriyorki, kırık kaynama işlemi esnasında mezenkim hücrelerinin kondroblastlar dönüşmesi için hücre içi biyolojik ortamın, kırığın oluştuğu ortamın veya diğer bazı bilinmeyen faktörlerin önemli etkisi vardır. Greft olarak nakledilen mezenkim dokusu gecikmelide olsa kallus dokusu ile bütünleşir ve kollusun nitelik ve nicelik olarak gelişmesine katkıda bulunur. Nakledilen mezenkim dokusuna karşı gelişen immun reaksiyon bu araştırmanın kapsamı dışında olsa da sonucu etkilemesi beklenen bir diğer önemli faktördür. Sonuç olarak, embriyonel mezenkim hücrelerinin kemik defektlerinin doldurulmasında etkili olduğu, ancak doku reddinin değerlendirilmesi için daha uzun süreli takibe ihtiyaç duyulduğu kanaatine varılmıştır.
