Abstract
It is complicated to treat delayed pediatric. Monteggia type fracture-dislocations. A seven year-old male patient of open Monteggia fracture-dislocation was initially treated by closed reduction in an urban hospital. With the development of tetanus infection the reduction became ineffective. A different course of treatment was then carried out. At the first session we have performed gradual distraction by an external fixator which is used for epiphyseal distraction in our clinic. As a conseauence redial head brought to the level of capitellum and fractured segments of ulna were appositioned. At the secont session, radial head was reduced and osteosynthesis of ulna was performed.
Özet
Gecikmiş çocuk Monteggia kırıklı çıkıklarının tedavisi sorunludur. Bu çalışmada bir taşra Devlet Hastanesinde, kapalı redüksiyon ile tedavi edilen ve tetanoz infeksiyonu gelişimi sebebiyle 1 ay reanimasyon servisinde yatan redüksiyonu bozulmuş ve ileri derecede deplase olmuş yedi yaşında bir çocuk açık Monteggia kırığının kliniğimizde değişik bir yaklaşımla tedavisi sunuldu. İIk seansta, kliniğimizde epifizyal distraksiyon amacıyla kullandığımız bir eksternal fiksatör ile tedrici distraksiyon sağlanarak, radius başı kapitellum hizasına, ulna uçları da karşı karşıya getirildi. İkinci seansta radius başının redüksiyonu ve ulnanın osteosentezi gerçekleştirildi. Yaklaşık beş yıl takip edilen olgumuzdan mükemmel klinik ve radyolojik sonuç aldık.
